Job Summary
Family Mentor role within Equip's Care Operations team delivering caregiver-focused sessions as part of a multidisciplinary care team. Responsibilities include providing skills-based individual and group sessions to caregivers, implementing motivational strategies to enhance engagement with care plans, participating in care-team huddles, proactively contacting families, conducting outreach and follow-up to ensure full engagement with treatment, personalizing care plans, coordinating services (e.g., scheduling, grocery lists, referrals), guiding caregivers in advocacy-related tasks (e.g., 504 Plan letters), maintaining documentation, and contributing to ongoing trainings. Requires 2+ years in healthcare-family-facing roles, lived experience with eating-disorder caregiving, strong communication and organizational skills, ability to work remotely with camera-enabled setup, comfort with ambiguity, and a passion for evidence-based care. Compensation ranges from $54,000 to $65,000 plus bonus, with comprehensive benefits and a fully remote work arrangement.
Required Qualifications
- 2+ years of experience working with families/supports in a health care setting including, but not limited to, coaching, case management, mentorship, social work, or milieu therapy
- Lived experience as an eating disorder support for a loved one in firm/stable recovery
- Ability to connect with a range of diverse people with varying lived experiences
- Ability to identify and direct patients and families in achievement of treatment goals
- Strong oral and written communication skills with the ability to build effective, professional, and collaborative relationships both internally and externally
- Strong organizational and time management skills with a proven track record of meeting goals
- Comfortable working remotely (including the requirement to be on camera)
- Comfortable with ambiguity and ability to think critically to quickly and accurately work through challenges in a fast-paced, fluid environment
