Job Summary
Family Medicine/Primary Care physician sought to provide part-time primary medical care for PACE participants at Mercy LIFE Assisi House (Aston, PA) and Mercy Life Valley View (Elwyn, PA) in the greater Philadelphia area. Under medical director supervision, the PCP conducts assessments, develops and implements care plans with the interdisciplinary team, manages care in nursing home settings, coordinates referrals to specialists, oversees hospital admissions when needed, and ensures documentation and diagnostic coding accuracy. Requires active MD/DO license in PA, CDS and DEA registration, BLS, internal medicine or family practice board certification (geriatrics certification preferred), willingness to learn American Sign Language, and minimum one year of experience with frail or elderly populations. Travel within the center’s geographic region and strong communication skills are required. Schedule includes two 8-hour days at Assisi House and one 4-hour day at Valley View with flexible weekdays; no night, weekend, on-call, or holiday coverage.
Required Qualifications
- MD or DO with plenary license to practice in state of PO
- CDS Certification and DEA Registration
- Current BLS
- Board Certification Internal Medicine or Family Practice
- Board Certification in Geriatrics preferred
- Willingness to learn American Sign Language
- Minimum of one year experience with frail or elderly population
- Valid driver’s license and use of personal vehicle
- Proficiency with Microsoft Office
- Strong communication and organizational skills
- Ability to adapt to changing priorities
- Ability to provide regular visits and care coordination in nursing home settings
