Job Summary
Primary Care Physician at Oak Street Health providing value-based, equitable care to Medicare patients in a local clinic setting. Responsibilities include assessing/diagnosing patients, overseeing and directing the center care team, prescribing medications, maintaining patient EMR data (Canopy/Greenway), and collaborating with regional leadership to meet health quality goals. Role supports outpatient, family- and internal-medicine scope within a value-based model; new graduates are encouraged to apply. Relocation may be considered case-by-case. Required Qualifications
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
- Internal Medicine or Family Medicine Board Certification (Or board eligible)
- Active, non-probationary, unrestricted State License
- Active DEA license
- US work authorization
