Family Medicine, Virtual Primary Care Physician

Primary Care Physician at Oak Street Health delivering equitable value-based care to Medicare patients in a neighborhood primary care setting. Responsibilities include assessing and diagnosing patients at local clinics, overseeing and administering primary care, prescribing and managing medications, maintaining Patient Electronic Medical Records, collaborating with regional/central leadership to meet quality goals, and directing the center care team with Practice Managers. Ideal candidates are MDs or DOs board-certified in Internal Medicine or Family Medicine (board eligible considered), hold an active state license and DEA, and are committed to outpatient care and geriatrics within a value-based healthcare model.
