Primary Care Physician for Oak Street Health/N Medicare patients in a neighborhood primary care network. The physician provides equitable, effective value-based care at local clinics, operating within a team-based model to deliver high-quality, preventive primary care with bonuses tied to outcomes. Responsibilities include assessing and diagnosing patients at local clinics, overseeing and administering primary care, prescribing and managing medications, maintaining Patient EMR data (Canopy and Greenway), collaborating with regional/central leadership to meet health quality goals, and working with Practice Managers to direct the center care team. New graduates are encouraged to apply; opportunities may include Center Medical Director responsibilities. Required qualifications include an MD or DO, board certification (or eligibility) in Internal Medicine or Family Medicine, an active unrestricted medical license, an active DEA license, and US work authorization. The role is full-time with anticipated weekly hours around 40 and a pay range of $174,070–$374,920, with benefits and a value-based compensation model. Beneficial experiences include geriatrics fellowship, outpatient primary care experience, and experience in value-based care; bilingual proficiency is valued where applicable.
