Family Medicine, Virtual Primary Care Physician

Provide equitable, value-based primary care to Medicare patients at Oak Street Health's neighborhood centers. Assess and diagnose patients, oversee, direct, and administer primary care, prescribe and administer medications, maintain Patient Electronic Medical Record data via canopy and greenway, and collaborate with regional/central leadership to meet health quality goals. Work with Practice Managers to direct and manage the center care team; new graduates encouraged to apply; potential Center Medical Director role for qualified candidates.
