Job Summary
Provide primary care services to patients in traditional practice settings, patients' homes, and via online appointments within a team-based model. Manage outpatient care with support from highly trained staff handling walk-in and overflow cases, utilizing an optimized EMR with full telehealth capability. Collaborate with cross-trained care management and population health teams to drive value-based care and reduce total cost of care. Maintain current state licensure, Board certification in Family Medicine or Internal Medicine, and active Medicare/Medicaid enrollment. Join the Lithonia, GA practice as a Board-Certified/Board Eligible physician.
Required Qualifications
- Board-Certified/Board Eligible Family or Internal Medicine Physicians
- 2+ years of clinical outpatient experience, post residency
- Current, unrestricted State license or eligibility for licensure
- Board certification/eligibility in Family Medicine or Internal Medicine
- Certification in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Active registration or eligibility to register with the Drug Enforcement Agency (DEA)
- Active Medicare/Medicaid enrollment or eligibility to enroll
- Comfortable with Electronic Medical Record (EMR) system
