Job Summary
Conduct primary care visits for patients, developing individual health and wellness plans through strong interpersonal engagement. Manage a whole-person, team-based approach that includes integrated behavioral health, dieticians, clinical pharmacists, social workers, and chaplains within the same office visit. Serve vulnerable populations as part of a faith-based environment while collaborating with over 100 community partners to extend support beyond clinical care. Board-certified Family Physicians with primary care experience are required.
