Job Summary
Conduct comprehensive primary care for underserved populations within a faith-based, Level 3 Patient-Centered Medical Home. Manage individualized health and wellness plans for patients while coordinating integrated behavioral health, dieticians, clinical pharmacists, and social workers during office visits. Serve as a medical director for experienced candidates and collaborate with over 100 community partners to deliver whole-person care. Board-certified Family Physicians with strong interpersonal skills are required. California residency is mandatory upon start date; this position is ineligible for a Conrad 30 Waiver.
