Job Summary
Conduct primary care visits for patients, developing individualized health and wellness plans through strong interpersonal relationships. Deliver integrated care by coordinating with behavioral health specialists, dieticians, clinical pharmacists, social workers, and chaplains within the same office visit. Serve vulnerable populations as part of a whole-person, team-based model supported by over 100 community partners. Candidates must be Board-Certified Family Physicians with primary care experience. Position offers a medical directorship opportunity for experienced providers in a faith-based, collegial environment.
