Job Summary
Conduct primary care visits for underserved populations within a faith-based, Level 3 Patient-Centered Medical Home environment. Develop individualized health and wellness plans for patients while coordinating integrated care with behavioral health, dieticians, pharmacists, and social workers in a single visit. Manage a caseload of 900+ medical residents and students, supporting their leadership roles in the community. Maintain strong interpersonal relationships to foster patient trust and collaborate with over 100 community partners to extend support beyond standard medical services.
