Job Summary
Conduct comprehensive primary care for underserved populations within a faith-based, whole-person model that integrates behavioral health, dieticians, pharmacists, and social workers. Build individualized health plans for each patient while collaborating with over 100 community partners to extend support beyond clinical care. Maintain Level-3 Patient-Centered Medical Home standards and provide integrated services including chaplaincy within the same office visit. Board certification and primary care experience are required. This role offers a salary of $285,000-$340,000, includes relocation assistance, and requires California residency.
