Job Summary
Conduct primary care for underserved populations in a faith-based, Level-3 Patient-Centered Medical Home environment. Deliver whole-person care by integrating behavioral health, dieticians, clinical pharmacists, social workers, and chaplains within single office visits. Build strong partnerships with over 100 community organizations to support vulnerable residents. Work with patients on individualized health and wellness plans to improve outcomes. Requires Board Certification in Family Medicine with primary care experience. Located in Barstow, CA, with a salary range of $285,000 to $340,000.
