Job Summary
Conduct primary care visits and develop individualized health and wellness plans for patients in a faith-based environment. Build strong relationships with underserved populations while delivering integrated care that includes behavioral health, dieticians, and social workers within a single office visit. Collaborate with over 100 community partners and Loma Linda University faculty to provide whole-person care at a Level 3 Patient-Centered Medical Home. Board certification and primary care experience are required.
