Job Summary
Conduct comprehensive primary care for underserved populations within a faith-based, Level-3 Patient-Centered Medical Home environment. Manage individualized health and wellness plans for patients while collaborating with integrated behavioral health, dieticians, clinical pharmacists, and social workers. Maintain strong partnerships with over 100 community organizations to deliver whole-person care to the surrounding 4 million-person region. Serve as a Board Certified Family Physician with primary care experience in a collegial, growth-oriented setting at SAC Health System.
