Job Summary
Conduct primary care for underserved populations within a faith-based, Level-3 Patient-Centered Medical Home. Deliver whole-person care by integrating behavioral health, dieticians, clinical pharmacists, social workers, and chaplains during office visits. Build individualized health plans for patients while collaborating with over 100 community partners to extend support beyond clinical boundaries. Board Certified Family Physician with strong interpersonal skills required to serve a surrounding population of nearly 4 million. California residency required upon start date.
