Job Summary
Conduct primary care for underserved populations within a faith-based, Level 3 Patient-Centered Medical Home. Deliver whole-person care by integrating behavioral health, dieticians, clinical pharmacists, social workers, and chaplains into single office visits. Build individualized health plans with patients while serving over 900 medical residents and students from Loma Linda University. Partner with more than 100 community organizations to support vulnerable populations in the Inland Empire. Board certification and primary care experience required.
