Job Summary
Conduct primary care for vulnerable populations in a faith-based, Level 3 Patient-Centered Medical Home environment. Develop individualized health and wellness plans for each patient while collaborating with integrated behavioral health, dieticians, pharmacists, social workers, and chaplains within the same visit. Serve as a Board-Certified Family Physician with strong interpersonal skills, working alongside over 900 medical residents and students to support underserved communities. Leverage partnerships with more than 100 community organizations to deliver whole-person care. Requires California residency; starting base compensation ranges from $255,000 to $310,000.
