Conduct primary care visits and develop individualized health and wellness plans for patients within a Level 3 Patient-Centered Medical Home. Manage whole-person care by coordinating integrated behavioral health, dieticians, clinical pharmacists, social workers, and chaplains during office visits. Serve over 900+ medical residents and students from Loma Linda University while partnering with more than 100 community organizations to support underserved populations. Join a faith-based environment in San Bernardino with a starting base compensation of $255,000–$310,000, subject to experience and faculty rank.
