Job Summary
Conduct comprehensive primary care for underserved populations within a Level-3 Patient-Centered Medical Home model. Deliver whole-person care by integrating behavioral health, dieticians, clinical pharmacists, social workers, and chaplains during office visits. Build individualized health and wellness plans for patients while serving over 900 medical residents and students from Loma Linda University. Collaborate with more than 100 community partners to extend support beyond clinical services.
