Job Summary
Family Medicine Physician needed in Pomona, CA to provide comprehensive primary care across the age continuum in a patient-centered medical model. Responsibilities include diagnosing, treating, and managing acute and chronic conditions; conducting medical history reviews, physical exams, and care planning; ordering and reviewing labs/imaging; prescribing medications and immunizations; coordinating specialty referrals and follow-up care; supporting preventive care and chronic disease management; collaborating with care teams to improve patient outcomes; participating in quality improvement and peer reviews; supervising Nurse Practitioners, Physician Assistants, and other clinical staff; and contributing to workflow improvements, patient engagement strategies. Includes after-hours on-call on a rotating basis; multiple approved clinical service locations within the Pomona area; productivity expectations (8.5–10 patients per 4-hour session). Required Qualifications
- MD or DO from an accredited medical school
- Active and unrestricted California Physician License
- Board Certified in Family Medicine (required)
- Active DEA License
- Current BLS Certification (AHA approved)
- Experience with Electronic Health Records (EHR)
- Strong clinical judgment across acute and chronic care management
- Ability to work effectively with diverse patient populations
- Strong leadership and team collaboration skills
- Spanish/English bilingual preferred
