Job Summary
Family Medicine Physician needed in Pomona, CA to provide comprehensive primary care across the age continuum in a patient-centered medical model. Responsibilities include diagnosing, treating, and managing acute and chronic conditions; performing history and exams; ordering labs/imaging; prescribing medications and immunizations; coordinating referrals and follow-ups; supporting preventative care and chronic disease management; supervising clinical staff; contributing to quality improvement and patient engagement initiatives; and participating in rotating after-hours on-call coverage. Requires MD/DO, CA license, Board Certification in Family Medicine, DEA and BLS, EHR experience, and strong teamwork and leadership. Bilingual Spanish/English preferred. Multiple approved clinical service locations within the Pomona area; after-hours call scheduled. Required Qualifications
- MD or DO from an accredited medical school
- Active and unrestricted California Physician License
- Board Certified in Family Medicine
- Active DEA License
- Current BLS Certification (AHA approved)
- Experience with Electronic Health Records (EHR)
- Strong clinical judgment across acute and chronic care management
- Ability to work effectively with diverse patient populations
- Strong leadership and team collaboration skills
- Spanish/English bilingual preferred
