Job Summary
Conduct comprehensive primary care for a culturally and ethnically diverse, medically complex patient population, including all ages. Collaborate with an embedded APP, behavioral health provider, clinical pharmacist, diabetes educator, and case management team to support panel and In Basket management. Practice trauma-informed care aligned with the mission of caring for the poor and vulnerable. Work a full-time schedule of 4 to 5 days per week (Monday to Friday) with flexible hours between 0.75 and 1.0 FTE.
Required Qualifications
- Board-certified or board-eligible Family Medicine
- Must be comfortable and willing to see all ages
- Must be able to practice with trauma-informed care
- Must be willing to honor the Providence mission of caring for the poor & the vulnerable
