Family Medicine Physician

Conduct comprehensive primary care for a culturally and ethnically diverse, medically complex patient population across all ages. Collaborate with an embedded APP, behavioral health provider, clinical pharmacist, diabetes educator, and case management team to support panel and In Basket management. Practice trauma-informed care aligned with the mission of serving the poor and vulnerable. Maintain a 4 to 5 days per week schedule Monday through Friday within a full-time flexible 0.75 to 1.0 FTE role.
