Family Medicine Physician - Post-Acute Care

Conduct daily rounding on 1-2 buildings per day, Monday through Friday, while managing a patient load of 20-22 patients. Provide comprehensive care to skilled, short-term patients with three visits per week, utilizing the Nursing Home EMR for efficient charting and receiving NP support. Attend to 3-5 buildings as needed and cover day call rotations from 8 AM to 6 PM for facilities rounded on that day. Earn a competitive annual salary of $180K-$220K with a productivity bonus, 15 days of PTO, and a $2,500 CME stipend.
