Job Summary
Conduct primary care services for underserved populations within a faith-based, Level-3 Patient-Centered Medical Home environment. Manage the Medical Director role at SAC Health System, leading a team that integrates behavioral health, dieticians, clinical pharmacists, and social workers into single office visits. Cultivate strong partnerships with over 100 community organizations to support whole-person care for vulnerable individuals. Board Certified Family Physicians with primary care experience are required. Residency is required upon start date.
