Job Summary
Conduct primary care services within a Level-3 Patient-Centered Medical Home, delivering integrated behavioral health, dietetics, clinical pharmacy, and chaplaincy support during single office visits. Manage patient relationships to develop individualized health and wellness plans, while overseeing the care of over 900 medical residents and students from Loma Linda University. Collaborate with more than 100 community partners to extend support for underserved populations in Indio, CA. Maintain Board Certification in Family Medicine with primary care experience, operating in a faith-based environment where California residency is required upon start date.
