Job Summary
Conduct comprehensive primary care for underserved populations within a faith-based, Level-3 Patient-Centered Medical Home environment. Manage patient relationships to develop individualized health and wellness plans, while overseeing care for over 900 medical residents and students. Lead a team-based approach integrating behavioral health, dietetics, clinical pharmacists, social workers, and chaplaincy services during single office visits. Collaborate with more than 100 community partners to extend support beyond clinical work. Requires Board Certification and California residency; compensation ranges from $285,000 to $340,000.
