Job Summary
Conduct comprehensive primary care services for underserved populations within a faith-based, Level-3 Patient-Centered Medical Home environment. Manage clinical operations and patient relationships to develop individualized health and wellness plans, while collaborating with integrated behavioral health, dieticians, pharmacists, and chaplains for whole-person care. Serve as Medical Director for the Department of Family Medicine at SAC Health System, supporting over 900 medical residents and students in a collegial setting. Board certification and primary care experience are required. California residency is mandatory upon start date.
