Job Summary
Per Diem adult and senior care physicians to join the post-acute care team across skilled nursing facilities in the Cumberland region. Provides direct patient care across the aging continuum including skilled nursing, rehab, personal care/assisted living, independent living and clinic. Interacts with facility leadership, hospital discharge team, care management and other specialties to drive high-quality care with aligned goals. Post-acute care offers diverse fields from general primary care and internal medicine to geriatrics and acute rehab; clinicians cross over with several specialists. Collaborates with and supervises Advanced Practice Providers. Uses EPIC EMR with online diagnostics including PACS imaging and laboratory results. Clinical rounding Monday through Friday with attractive daily schedules; overnight and weekend call shared with partners and APPs. Eligible for PA licensure. Required Qualifications
- Board certified/Board eligible in Internal, Family or Geriatric Medicine
- Eligible for licensure in the state of Pennsylvania
- Per diem/ad hoc scheduling experience in post-acute or geriatric settings
- Proficient with EPIC EMR, PACS imaging, and laboratory results interpretation
- Direct patient care across multiple facilities in aging continuum
- Ability to collaborate with facility leadership, discharge team, and care management
- Supervision of Advanced Practice Providers (APPs)
