Provide direct patient care in skilled nursing, rehab, personal care/assisted living, independent living, and clinic facilities across the aging continuum. Interact with facility leadership, hospital discharge teams, and care management to drive optimal quality care with aligned goals. Collaborate with and supervise Advanced Practice Providers while utilizing EPIC EMR, online diagnostics, PACS imaging, and laboratory results. Perform clinical rounding Monday through Friday with flexible daily schedules; overnight and weekend call consists of all-phone coverage shared equally with partners and APPs. Board certified or eligible in Internal, Family, or Geriatric Medicine with Pennsylvania licensure.
