Job Summary
Provide comprehensive primary care services to patients of all ages, diagnosing and managing acute and chronic conditions while performing preventive care and wellness visits. Deliver continuity of care across life stages by collaborating with nurse practitioners and physician assistants to ensure coordinated treatment. Utilize electronic health records and telehealth tools to enhance access, and educate patients on health promotion and disease prevention. Participate in quality improvement initiatives and practice development efforts. This role supports a collaborative, team-based model within a Patient-Centered Medical Home serving the community since 1998.
Required Qualifications
- completion of Doctor of Osteopathy (DO) or Doctor of Medicine (MD)
- completion of an accredited internship and residency
- active license to practice medicine in NYS
- currently board certified by an American Board of Medical Specialties (ABMS) Member Board or an approved American Osteopathic Association (AOA) specialty board
- board eligible status
