Family Medicine, Medical Doctor / Primary Care Physician

Assess and diagnose patients at local clinics while overseeing, directing, and administering primary care for Medicare populations. Prescribe pharmaceutical treatments and maintain Patient Electronic Medical Record data via Canopy and Greenway. Collaborate with regional leadership to meet health quality goals and work with Practice Managers to direct and manage the center care team. Other duties as assigned. This role operates within a value-based care model focused on outcomes, low hospital admissions, and preventive medicine for older adults.
