Job Summary
Family Medicine Physician needed in northwest Indiana (Crown Point, Valparaiso, and nearby communities) for a small-group, Monday–Friday practice with no weekends. Clinicians work 36 hours per week with an average daily census of 20–24 and a shared call structure. The role uses EPIC EMR and includes a guaranteed base salary around $250K with RVUs and a productivity/quality bonus. Relocation assistance up to $25,000 and a robust benefits package (medical, dental, vision, malpractice with tail, license/dues, CME) are provided. Located within an hour’s drive of Chicago, in Crown Point, Valparaiso, and Carmel, IN.
