Job Summary
Conduct home visits with a caseload of up to 25 families, delivering parenting education, child development guidance, and safety planning while maintaining ongoing contact between visits. Connect families to community resources for early childhood education, healthcare, housing, and mental health services, documenting referrals and confirming successful connections. Recruit eligible families through outreach to community organizations and faith networks, then screen for TANF eligibility and coordinate initial assessments with the Public Health Nurse. Plan and facilitate monthly workshops on child development and financial literacy, managing childcare and refreshments to support participation. Maintain accurate case management records, support TANF documentation, and update individualized family service plans quarterly.
Required Qualifications
- Bachelor's degree in Social Work, Human Services, Early Childhood, or a related field preferred; equivalent combination of education and experience will be considered
- Minimum 1-2 years of experience in child and family development, home visiting, case management, or a related direct-service role
- Valid MN Driver's License and access to reliable transportation
- Must pass a criminal background check
