Job Summary
Family Home Provider (FHP) contract role with Southwest Center. Providers create a supportive home environment for individuals with intellectual and developmental disabilities, assisting with daily living, transportation, community participation, and personal growth goals while promoting independence, dignity, safety, and meaningful community involvement. Responsibilities include maintaining a safe/respectful home, supporting person-centered goals, collaboration with the individual’s support team, and professional communication about services. The approval process includes a home assessment and provider review, with multiple meetings/visits to ensure suitability. Required documentation includes provider application and onboarding paperwork, identity/work authorization verification, physician medical verification, TB risk assessment, criminal background checks, driver’s license and vehicle insurance documentation for household drivers, and drug screening. Training and ongoing oversight are provided by Southwest Center during onboarding and certification.
Required Qualifications
- Contract provider approval and certification process
- Home-based setting experience preferred? (as per description: supports individuals with disabilities)
- Driver’s license and vehicle insurance (for household drivers)
