Family Finding Case Manager

Job Summary

Communicate with assigned DHS or CUA workers via phone and writing to obtain accurate case backgrounds and documentation. Visit with youth or make monthly phone contact, ensuring in-person intake occurs barring unusual circumstances. Send letters to required parties by the 15th day of service and follow up as needed. Provide ongoing identified supports specific to each family and complete detailed reports including background, placement details, and court history. Maintain timely supervision notes documenting progress, next steps, and communication with the team. Adhere to all confidentiality standards while utilizing solution-focused questioning techniques to connect children with their families. This hybrid position requires a Bachelor's degree in social work, two years of child welfare experience, and current Pennsylvania clearances.

Required Qualifications

  • Knowledge of social services, child welfare, and family systems services
  • Strong writing and oral presentation skills
  • Strong organizational skills, an ability to multitask, and prioritize
  • Strong interpersonal skills, respectful, and courteous nature
  • Able to work independently, demonstrating good judgment
  • Minimum of two (2) years of experience in child welfare
  • demonstrated knowledge of child, adolescent, adult, and family functioning
  • the ability to build effective relationships with families, community partners, and internal/external stakeholders
  • Experience as an Adoption Worker or Foster Home Certification Worker
  • Current Pennsylvania Child Abuse History, Criminal History, and FBI clearances
  • Bachelor's degree in social work or a related field
