Job Summary
Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and Clinical Supervisor. Provide individual, group, and family therapeutic interventions, as well as skill-building outlined in the Treatment Plan. Attend weekly Treatment Team meetings and link clients with specific services and resources identified in their plans. Serve as a liaison with other professionals, agencies, and community resources to ensure comprehensive needs are met. Maintain accurate and timely documentation of all client interactions, treatment plans, and progress notes in compliance with organizational policy and regulatory standards. Actively recruit new referrals to maintain a full caseload and provide 24/7 on-call support for clients as required by the program. Ensure compliance with all state regulatory bodies and COA standards while participating in ongoing training and professional development activities.
Required Qualifications
- Bachelor's degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field
Desired Qualifications
- Minimum of two years experience working with children and/or families. This experience may include internships and volunteer work.
