Job Summary
Develop individualized treatment plans and coordinate care for clients and families with treatment team input. Provide individual, group, and family therapeutic interventions, as well as skill-building outlined in the Treatment Plan. Attend weekly Treatment Team meetings and link clients with specific services and resources identified in their plans. Serve as a liaison with professionals, agencies, and community resources to ensure comprehensive needs are met. Maintain accurate documentation of all client interactions and progress notes in compliance with regulatory standards. Actively recruit new referrals to maintain a full caseload and provide 24/7 on-call support as required. Ensure compliance with all state regulatory bodies and COA standards while participating in ongoing training for quality service delivery.
Required Qualifications
- Bachelor's degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field
- Minimum of two years experience working with children and/or families
- Experience may include internships and volunteer work
