Job Summary
Coordinate intensive home-based services and provide crisis intervention for children and families within the Flexible Integrated Treatment program. Collaborate with therapists and community partners to facilitate Child and Family Team meetings, manage case coordination, and deliver in-school assistance, mentoring, and parental guidance. Safely transport clients to appointments and respond to after-hours on-call coverage for immediate intervention. Empower families to meet their goals while maintaining strict attendance standards and completing required paperwork.
Required Qualifications
- Willingness to take a whatever it takes approach to supporting the family needs
- Responsible for establishing a positive rapport with high needs families
- Works cooperatively and effectively with Therapist
- Collaborates with the treatment team to best serve the client and family
- Effectively coordinates with community partners and resources to increase client and family supports
- Demonstrates and understands / interest in strength based strategies for supporting the family
- Ability to work independently
- Manage a regularly changing daily schedule
- Ability to effectively communicate, collaborate and partner with community partners and resources
- Safely transports clients and other passengers to and from appointments and activities using own personal vehicle
- Provides prompt intervention in the event of a crisis
- Attends to the health, safety, and well-being of child and family
- Provides after hours, on-call coverage to program clients
- Meets the standard for consistent attendance (not more than a 5% absence rate per month)
- Graduation from a four-year college or university with a Bachelors degree in Social Work, Psychology, or related field
- Varied experience as a provider of mental health services
- Experience working with children and families
Desired Qualifications
- Additional qualifying experience may be substituted for education
