Job Summary
Provide in-home nursing visits to families with newborns in Tulsa under the Family Connects program. Follow the Family Connects home-visiting protocol, perform health and physical assessments of mother and baby in the home, evaluate family needs across 12 domains of physical and psychosocial well-being, document observations in real-time, and facilitate referrals to community resources. Build trusting relationships with families, collaborate with a multi-disciplinary team, assist families in scheduling and attending doctor visits and appointments, and advocate for families as needed. Apply knowledge of parent–child interaction, child-maternal health, child development, and child abuse/neglect to support parenting skills and family health plans. Maintain home-visit schedules, complete timely documentation, participate in team meetings and quality initiatives, and uphold agency values. Requires RN licensure (or equivalent nursing credential with RN license), Spanish fluency, CPR, a valid driver's license, ability to lift 50 pounds, and a flexible schedule.
Required Qualifications
- B.S. in nursing with current state RN license, or B.A. in another field with current RN license, or A.S. in nursing with state RN license
- CPR certification
- Fluency in Spanish (read, write, speak)
- Valid state driver’s license and automobile for home visits
- Ability to move or lift at least 50 pounds
- Ability to work a flexible yet demanding schedule
- Proficiency in Microsoft Office and ability to learn new technology quickly
