Job Summary

Deliver Family Centered Treatment clinical services to children, adolescents, and families primarily in home and community environments, coordinating with treatment teams to establish goals and manage discharge plans. Conduct interventions aligned with FCT model fidelity requirements, provide psychoeducation, and ensure timely completion of clinical records and internal training checkoffs. Maintain 24/7 on-call coverage for a caseload, requiring a flexible schedule to meet family convenience. Collaborate with medical and non-medical providers to coordinate community resources and oversee initial and ongoing assessment activities.

Required Qualifications

  • Minimum of Master's Degree in Counseling, Social Work, or related field
  • Two years' experience working with youth or families
  • Must hold one of the following current professional licenses in the state of Ohio: LSW, LISW, LPC, LMFT, or LPCC
  • Must be able to provide 24/7 on-call coverage to a caseload of families and children
  • Must be able to work a flexible work schedule
  • Must be able to deliver FCT clinical services to children, adolescents, and their families, primarily in their living environments
  • Must be able to complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record
  • Must be able to develop Service Plan in coordination with Treatment Team Process
  • Must be able to provide psychoeducation as indicated in the Service Plan
  • Must be able to conduct interventions with the family aligned with the FCT model
  • Must be able to assist the families and children to establish weekly goals
  • Must be able to develop and coordinate discharge plan based on treatment
  • Must be able to coordinate and oversee the initial and ongoing assessment activities
  • Must be able to convene the Treatment Team for Service Plan
  • Must be able to consult with identified medical and non-medical providers
  • Must be able to provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members
  • Must be able to ensure that your credentials remain current
  • Must be able to comply with all training requirements
  • Must be able to adequately supervise your staff
  • Must be able to provide Family Centered Treatment (FCT) Clinical Services as per the service definition and model fidelity requirements
  • Must be able to complete FCT Checkoffs and other internal training within designated deadlines
  • Must be able to access community resources that would be beneficial to client aftercare needs
