Job Summary
Deliver structured, in-home family therapy to children, adolescents, and families using the Family Centered Treatment model with strict fidelity and engagement requirements. Develop and adjust service plans in collaboration with treatment teams and coordinate care across schools, medical providers, and child welfare systems to ensure integrated delivery. Maintain accurate documentation, complete required FCT forms, and support discharge planning and aftercare coordination. This role requires 24/7 on-call availability and completion of FCT certification within the first year.
Required Qualifications
- Minimum of a Master's Degree in Social Work, Counseling, or related field
- Two years' experience working with youth or families
- Current Family Centered Treatment Certification is preferred
