Family Centered Treatment Specialist

Deliver Family Centered Treatment services primarily in home and community settings, guiding children, adolescents, and families through structured, research-backed treatment phases. Serve as the central point of coordination for each family's Treatment Team, collaborating with medical, behavioral health, school, and community partners to ensure seamless, whole-person care. Develop, implement, and refine individualized Service Plans in partnership with families while maintaining timely documentation in the Electronic Health Record and participating in Quality Assurance activities. Connect families to community resources, referrals, and aftercare supports that extend the impact of treatment beyond direct sessions. This role requires a Bachelor's degree, two years of experience with youth or families, and FCT certification completed within one year of hire. The position includes a flexible schedule with 24/7 on-call availability for crisis support.
