Family Case Planner

Job Summary

Conduct initial assessments of family needs and placement risks to develop preliminary service plans in coordination with supervisors. Deliver family counseling, case planning, and preventive services while monitoring linkages with community providers and securing child safety. Facilitate access to psychiatric services, follow clinician recommendations, and maintain at least three monthly face-to-face contacts including home and office visits. Execute administrative tasks such as completing FASPS, progress notes, and contact logs while participating in weekly supervision and leading parenting or adolescent groups.

Required Qualifications

  • BA/BS/BSW degree
  • Strong oral and written communication skills
  • Ability to understand sensitive information and maintain confidentiality
  • Strong interpersonal and communication skills at all levels of the organization
  • Strong critical thinking, complex problem solving skills
  • Highly organized, detail oriented and able to manage multiple tasks simultaneously
  • Proficient computer knowledge, including word processing and database software
  • Ability to use public transportation for travel to program locations throughout New York City
  • Must maintain confidentiality and have the ability to exercise a high level of judgment/discretion
  • Ability to create and maintain well-organized administrative and operational systems
  • Knowledge and proficiency of MS Word, Excel, PowerPoint
  • Ability to work independently and manage multiple tasks in a fast-paced environment
  • Ability to work collaboratively with a variety of people, as well as an aptitude for teamwork
  • Must be able to effectively lead and coach others
  • Must be able to lift and/or move up to 20 pounds

Desired Qualifications

  • BA/BS/BSW degree preferred
  • Interest in working with people with social service needs
  • Exceptional communication and organizational skills, including excellent phone demeanor and direct communication skills with program participants, vendors and staff
  • Ability to operate with purpose, urgency, and accuracy in a fast-paced deadline-driven environment
  • Ability to create and maintain well-organized administrative and operational systems
  • Knowledge and proficiency of MS Word, Excel, PowerPoint, and possess a willingness to learn new programs as needed
  • Spanish speaking a plus
