Job Summary
Complete timely and accurate documentation in the Case Record, including referral intake packets, entitlements, and related correspondences. Perform Catholic Charities Housing Office assessments for new tenants and annually thereafter. Develop and implement Individual Service Plans (ISPs), monitor their effectiveness every six months, and guide family members toward positive results. Maintain case notes documenting progress toward milestones and conduct case conferences with the Director of Multi-Family Housing, CCHO staff, DSS workers, and other treatment providers. Cooperate in ongoing supervisory processes with the DSS, attend regularly scheduled case management team meetings, and advocate for clients while escorting them to appointments as needed. Monday through Friday, 11:00am to 7:00pm.
Required Qualifications
- Bachelor's Degree in a related field
- 1 - 3 years of experience working with a diverse population
- Valid driver's license
- Proof of current car insurance
- Registration for a vehicle
- Access to a reliable, working vehicle
