Job Summary
Field-based LTSS Service Coordinator (Case Manager) in Wisconsin who conducts face-to-face program assessments, applies motivational interviewing, coordinates waivers (e.g., Family Care Partnership), and develops Person Centered Plans. Serves as single point of contact with clinical teams, manages non-clinical needs for members with chronic illnesses, documents short- and long-term goals with the member and care team, mentors staff, submits utilization/authorization requests, and participates in state audits and appeal processes. Requires a BA/BS and at least 2 years of social work experience; role involves travel to client sites with occasional office attendance; hybrid work arrangement within commuting distance from posting locations; vaccination requirements may apply for patient/member-facing roles. Locations include Milwaukee County and surrounding Wisconsin communities.
Required Qualifications
- BA/BS degree
- minimum of 2 years of experience working with a social work agency
- Travel to worksite and other locations as necessary

- Vaccination requirements for certain patient/member-facing roles
