Job Summary
The Family Care Coordinator provides care coordination services to youth clients with mental health and substance abuse issues and their families. Responsibilities include implementing monitoring systems, assessing client needs, conducting interviews for assessments, and ensuring delivery of necessary treatment and services. The role involves meeting with individual clients, facilitating group sessions, and coordinating care with internal and external providers while sharing on-call responsibilities.
Required Qualifications
- Associate’s Degree in Human Services or related field
- 1-3 years of case management experience for individuals with mental health or substance abuse issues
- State of Ohio Driver’s License
- BLS/CPR certification
Desired Qualifications
- Licensed Professional Counselor (LPC)
- Qualified Mental Health Specialist (QMHS)
- Strong written and oral communication skills
- Collaboration skills with other service providers
