Job Summary
Coordinate care planning teams to deliver medically necessary intensive family therapy, robust care coordination, and Family Partner engagement for youth with serious emotional disturbance in home and community settings. Serve as a liaison between individuals, partnering agencies, and supportive programs to facilitate care, identify and help families access available services and natural supports, and provide trauma-informed wraparound services that prioritize youth needs. Assist parents and caregivers in developing effective advocacy skills. Position covers the Merrimack Valley region with flexible afternoon/evening hours; bilingual bonus and professional development benefits are offered.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, or a related field is required; a Master's degree is preferred
- Fluency in Spanish and English is required
- A valid driver’s license and access to a vehicle
- Must be 21 years of age
