Job Summary
Provide high quality primary medical care in the outpatient primary care setting as an APRN, developing and executing a plan of care for each patient including history, exam, diagnosis, treatment and referrals as needed. Collaborate with clinicians and families, maintain accurate EPIC medical records, participate in staff meetings and performance-improvement activities, and pursue continuing education to ensure high-quality patient care. Must be licensed as an APRN in Connecticut; bilingual Spanish/English desirable; equal opportunity employer.
Required Qualifications
- Graduation from an accredited APRN program
- License to practice as an APRN in State of Connecticut
- Bi-lingual in Spanish and English highly desirable
